Prize Draw Terms and Conditions

Terms and Conditions of Entry into the Prize Draw

  1. The Promoter of this prize is Mary Gregory as part of her research obligations at the Queensland of the Sunshine Coast ABN 28 411 859 157 (USC) which has its principal place of business at 90 Sippy Down Drive, Sippy Downs, Queensland, 4556.
  2. By entering the competition connected to the research project named in the attached Participant Information and Consent Form, participants agree to be bound by these Terms and Conditions.
  3. Submitting both survey questions before and after the intervention as well as completing the intervention itself fulfils the conditions of entry into the competition.
  4. The opening and closing dates of the competition are listed on the attached Participant Information Sheet and Consent Form. The prize winner will be selected by a random draw which will take place at USC on the prize draw date as listed in the attached Participant Information Sheet and Consent Form.
  5. The first entry drawn randomly will receive the prize listed on the attached Participant Information Sheet. The prize is not transferable, refundable, exchangeable, and cannot be taken as cash.
  6. The Researcher will use reasonable efforts to notify the prize winner by no later than two weeks post the prize draw date. If a winner cannot be contacted or refuses the prize it will be offered to the next randomly drawn entry. The Researcher’s decision is final as to the winner of the prize and binding and no correspondence will be entered into in relation to the conduct of the competition or otherwise polska-ed.com.
  7. The prize will be available for collection by the winner at a time organised as suitable for the winner.
  8. An announcement will be sent to the winners email to notify them. No identifying details of the prize winner/s will be made public.
  9. If this competition is interfered with in any way or is not capable of being conducted as anticipated due to any reason beyond reasonable control of the Researcher, the Researcher reserves the right, in its sole discretion, to the fullest extent permitted by the law to (a) disqualify any participant; or (b) subject to any written directions from a regulatory authority, to modify, suspend, terminate or cancel the competition, as appropriate.
  10. All entries become the property of the Researcher and will not be returned to participants.
  11. Except for any liability that cannot be excluded by law, the Researcher excludes all liability (including negligence), for any personal injury or any loss or damage (including loss of opportunity), whether direct, indirect, special or consequential, arising in any way out of the competition, including but not limited to, where arising out of the following: (a) any technical difficulties or equipment malfunction (whether or not under the Researcher’s control); (b) any theft, unauthorised access or third party interference; (c) any entry or prize claim that is late, lost, altered, damaged or misdirected (whether or not after their receipt by the Researcher) due to any reason beyond the reasonable control of the Researcher; (d) any variation in prize value to that stated in these Terms and Conditions; (e) any tax liability incurred by a winner or entrant; or (f) use of the prize.
  12. Each participant indemnifies and keeps indemnified the Researcher against all claims, losses, damages and expenses suffered by the Researcher or any third parties arising out of the breach of these Terms and Conditions by the participant, the conduct of the participant in the competition, the use of information supplied by the participant or the use of the prize. Under the applicable Privacy Laws, the Researcher must tell participants when it collects personal information about them and how it plans to use it.  lf a participant chooses to enter or take part in the competition, the participant will be required to provide the Researcher with personal information such as the participant’s name and email address. The Researcher collects participants’ personal information in order to conduct the competition and this will not be linked to your answers in the research study.
